Transition Projects-a leader in transitioning people experiencing homelessness into housing in Portland, and Multnomah County, Oregon-was founded in 1969. The agency operates 9 unique locations and places hundreds of people into housing each year in the Metro-area. Transition Projects offers programs, resources and tools to individuals through access to social services including case management, shelter, basic needs, support services, mentorship and housing. A staff of 275 serves nearly 10,000 annually.
